La thèse est construite autour d’une double démarche de nature conceptuelle et méthodologique, l’une a pour objectif de tenter de répondre à la problématique suivante: quels sont les mécanismes tant pratiques que théoriques mettant en ‘jeu” le fonctionnement d’un système, l’institution, en tant qu'organisation structurée pour atteindre un objectif, le sien, et en même temps qui la spécifie et l’individualise en tant que telle, à l’intérieur d’un système plus vaste l’englobant, représenté par l’ordre social que nous désignons du terme de société ? La seconde démarche est axée autour d’une hypothèse contenue dans l’énoncé même de la problématique : toute institution sociale qui est en conformité aux critères d’organisation structurée pour un objectif spécifique est un système intégré à un ensemble d’autres systèmes sociaux et bien qu’étant autonome s’implique dans des relations de nature complémentaire et fonctionne en interdépendance dans cet ensemble qui constitue l’entité sociale et dans le cadre d’un objectif commun. Une telle approche s’inscrit dans un perspective dynamique dans la quelle la notion d’équilibre prend tout son relief en conditionnant tout le fonctionnement d’un système dont le modèle type sans être singulier est représenté par l’institution hospitalière, objet de cette étude.

The Thesis is constructed around a double step of conceptual and methodological nature, the one has as objective to try to answer following problems: which are practical or theoretical mechanisms throwing in game the functioning of a system, the institution, as organization structured to reach a target, and all at once which specifies it and individualizes it as such, inside a huger system including it, represented by social order than we indicate the term of society? The second step focuses in a hypothesis contained in the terms of the problem: any social institution which is in conformity with the criteria of organization structured for a specific objective is a system inserted into all other social systems and although being autonomous gets involved in relations of supplementary nature and works in interrelationship in this group which constitutes social entity and within the context of a common objective. One such approach registers in one dynamic perspective in which the notion of equilibrium takes all its relief by conditioning all functioning of a system the model type without being singular is represented by the hospital institution, object of this study.
