Une supervision centrée sur l'alliance : effets sur la réflexivité des psychothérapeutes et sur les représentations des patients de l'alliance

L’alliance prédit le succès thérapeutique. Également, la détérioration de cette dernière est liée à des impasses thérapeutiques et à l’abandon des séances. On observe que ces derniers sont moins fréquents lorsque le thérapeute est attentif aux processus relationnels qui entrent en jeu en thérapie. La supervision centrée sur l’alliance peut-elle contribuer à améliorer les capacités réflexives et relationnelles des thérapeutes dans l’alliance ? L’étude compare 15 thérapeutes ayant participé à cette supervision à 15 autres n’y ayant pas participé. Les résultats à l’Inventaire d’alliance de travail - complété par les thérapeutes et par leurs patients – montrent que la supervision centrée sur l’alliance améliore les représentations des thérapeutes et des patients quant à la capacité du thérapeute à installer le dispositif de soins dans un environnement intersubjectif fondé sur l’alliance. Les représentations des thérapeutes et des patients s’améliorent donc concernant la capacité du thérapeute à aider le patient, à le faire progresser et à l'amener à se comprendre. Les jeunes thérapeutes semblent capables de négocier l’alliance affective avec le patient, ce qui leur est plus difficile pour l'entente autour des objectifs thérapeutiques. Également, un point de vue davantage autonome sur l’alliance apparaît chez les patients et les thérapeutes ayant participé à la supervision centrée sur l’alliance que dans le groupe où les thérapeutes n’ont pas participé. Dans ce second groupe, l’alliance des patients se déploie d’une manière anaclitique, c’est-à-dire en s’étayant sur la représentation de l’alliance des thérapeutes. Les résultats à l’Évaluation multidimensionnelle de la régulation des émotions montrent également un effet protecteur de la supervision centrée sur l’alliance sur l’impulsivité des thérapeutes. Enfin, l’étude montre que les thérapeutes seniors identifient plus de mouvements de l’alliance que les jeunes thérapeutes. Globalement, le modèle de supervision apparaît utile pour aider le thérapeute à installer le dispositif de soins dans un environnement intersubjectif fondé sur l’alliance. Elle semble également avoir un effet protecteur sur leur impulsivité. Même si l’expérience professionnelle permettrait de développer la réflexivité et les capacités relationnelles, il paraît important de continuer à former les jeunes thérapeutes à l’alliance – en particulier à l'entente autour des objectifs thérapeutiques avec le patient.
The alliance predicts therapeutic success. Also, the deterioration of the latter is linked to therapeutic impasses and to the termination of sessions. Can alliance-focused supervision help improve reflexive and relational abilities of therapists in the alliance? Study compares 15 therapists who participated in this supervision with 15 who did not. Results at the Working Alliance Inventory - completed by therapists and patients - show that alliance-focused supervision improves therapists’ and patients' perceptions of the therapist's ability to install the care device in an intersubjective environment based on the alliance. Thus, the perceptions of therapists and patients is improving with respect to the therapist's ability to help the patient, to make him progress and to help him understand himself. Young therapists seem to be able to negotiate the affective alliance with the patient, which appear more difficult for them concerning the agreement around the therapeutic goals. Also, a more independent perspective on the alliance appears in the patients and therapists who participated in the alliance-focused supervision than in the group where the therapists did not participate. In this second group, the alliance of the patient unfolds in an anaclitic way, relying on therapists’ representation of the alliance. Results in the Difficulties in Emotion Regulation Scale also show a protective effect of the alliance-focused supervision on therapist impulsiveness. Finally, the study shows that senior therapists identify more alliance movements than less experienced therapists. Overall, alliance-focused supervision appears useful to help therapist install the care device in an intersubjective environment based on the alliance. It also seems to have a protective effect on their impulsiveness. Although professional experience seems to develop reflexivity and relational skills, in continuing professional development, it seems important to continue to train young therapists to the alliance – especially to the agreement around therapeutic goals with the patient.
